MySQL Insert in Dynamic Forms with jQuery And PHP

Good morning to all the community source code, I'm new on the web and this time I have a form with dynamic fields assigned by the user, useful to restrict the user not to Himself as static fields is the insertion of these fields MySQL, in the design of BD not know if it is the most optimal bright ideas, opinions and hope.

boss if can u please make the validation to multiple fields if empty or other .. then it will be very helpful

thank you all for the positive and negative comments, is just one example, in terms that we perform validation friend at the end of the project when it again validated have everything up
